We require all students undertaking professional teaching courses to complete an occupational health check. This initial screening form is to identify if further support from our Occupational Health service is required to ensure that new students meet the required health standards of their course before starting.
This initial screening form must be completed immediately so that, where applicable, we can ensure that you have support in place prior to the commencement of the course.
Your answers to this questionnaire are CONFIDENTIAL and will not be given to anyone without your written permission. For further information about how we process personal data, please see our student privacy notice.
Occupational health clearance is a condition of your enrolment therefore must be completed.
The Occupational Health Check will enable the University to make well-informed decisions about your fitness to train on your chosen course and to consider any reasonable adjustments that may be necessary to support you throughout your study, including your placements.
